Mad honey is the intentional product of bees foraging on toxic rhododendron species — primarily Rhododendron ponticum and R. luteum — in two regions where the producing plants grow at sufficient density to dominate the floral bloom: the eastern Black Sea coast of Turkey (particularly the Kaçkar Mountains around Trabzon and Rize provinces), and the Nepali Himalayas. The honey contains grayanotoxins, a class of diterpenoid neurotoxins that act on voltage-gated sodium channels in mammalian nervous tissue, at concentrations dramatically higher than the trace amounts present in standard culinary rhododendron honey from safe species.
Grayanotoxin poisoning (sometimes called "mad honey disease") is a documented clinical condition. Onset is typically within 30 minutes to several hours of ingestion. Cardiac effects — particularly bradycardia (slow heart rate) and hypotension (low blood pressure) — and neurologic effects are the documented hallmarks; severe cases require emergency medical care. The clinical record is substantial: case reports across decades of Turkish, Korean, European, and US emergency-medicine literature, plus systematic reviews including Erenler 2016 (Turkish cardiac effects review) and Aryal 2025 (mechanism and clinical management review).
The historical record is one of the deepest of any single honey product. Xenophon's Anabasis (c. 401 BCE) records the first documented mass mad-honey poisoning: when the Greek "Ten Thousand" mercenaries retreated through the Pontic Mountains, soldiers who consumed local honey near Trabzon became severely incapacitated within hours — vomiting, prostration, and disorientation lasting through the next day. The episode is one of the earliest detailed clinical descriptions of food-borne neurotoxicity in Western literature. Mithridates VI of Pontus (134–63 BCE) is reported to have used mad honey as a weapon against Roman forces under Pompey, leaving honey caches along Roman invasion routes that incapacitated soldiers who consumed them — a documented ancient instance of biological warfare. Deli bal persisted as a regional Turkish Black Sea product through the Byzantine and Ottoman periods and into modern Turkish trade.
In modern Turkey, mad honey is produced commercially in the Kaçkar Mountain region (Trabzon, Rize, Artvin provinces) and sold in Turkish markets as deli bal. In Nepal, mad honey is harvested partly from Apis laboriosa cliff colonies (overlapping with the Himalayan cliff honey tradition) and partly from managed apiaries near alpine rhododendron stands. South Korea banned mad honey imports in 2005 after a series of poisoning cases linked to Nepali imports; the European Union restricts mad honey under broader food-safety frameworks; the United States permits sale but the FDA classifies it as an unsafe food additive and prohibits health-claims marketing.
